Requisition Number 0606365 Position Summary: The Adoption Assistant for SWIFT/SNAP provides support to the Regional Adoption Specialist and Permanency Services Branch in the administration of SWIFT and Special Needs Adoption programs.
Responsibilities include the following: Coordinate/schedule SWIFT meetings with Regional Adoption Specialist, FSOS and Case Manager. Prepare SWIFT forms/packet prior to meetings. The SNAP referral packet consists of several things such as the Adopt USA form, the 84 Matching form, the TPR order, and a recent picture. The incumbent would assist the child's worker with gathering the information/assessment, and completing the form, as well as completing the narrative of the child. A face to face contact/home visit would be required. Enter SWIFT data in TWIST adoption screens.
Each child with the goal of adoption needs a SWIFT meeting at least quarterly. The updated information gathered at these meetings is entered into the TWIST system, including who was in attendance. Initial meetings are also required within 10 days of the goal change to adoption. The Assistant will attend meetings set by the Out of Home Care Specialist and enter the information. Compile needed paperwork/materials for Interstate Compact referral. Submit SNAP referral with all needed paperwork by gathering all necessary information to complete packets to Stacy Hait, Central Office, SNAP Program. Notify Regional Adoption Specialist when a child is referred to the SNAP program. Assist RAS in using individualized matching program to identify potential adoptive placements. Review Adoptive Home Studies with case worker, FSOS and Regional Adoption Specialist. Recruit appropriate children to the Special Needs Adoption Program. Recruitment events are held regularly generally in the Lexington and/or Louisville area and attendance will be required. The incumbent is required to organize and oversee events in the Two River Region. The assistant would be required to run adoption matches on TWIST quarterly which match children with families across the state. Send SNAP reports to the region monthly. Update/correct information regularly. Other SWIFT/SNAP duties as assigned.
Minimum Educational Qualifications: High school diploma or GED required. Minimum Experience Qualifications: Two years of related experience is required Desired Qualifications Bachelor's degree preferred.
